MyHealth Terms and Conditions
MyHealth is an Internet service that allows Harvard Vanguard Medical Associates patients to have secure web-based access to portions of their electronic medical record and to communicate with their Harvard Vanguard clinicians on non-urgentmatters.

Patient Guidelines and Terms and Conditions for Use
By accessing or using MyHealth you acknowledge and agree to comply with the guidelines stated below and all the terms and conditions of this Agreement including Harvard Vanguard's Notice of Privacy Practices. It is important to review this Agreement and our Privacy Statement periodically as we may modify this agreement and the Privacy Statement at any time, and you agree that such modifications are effective and binding upon you immediately upon posting of the modified version.
- You agree NOT TO SHARE your password with anyone. Sharing your password will enable others to have access to your personal confidential medical information. You are solely responsible for protecting your password.
- You agree not to send any messages requiring urgent attention. Delays can occur based upon volume, availability of staff and the complexity of your condition. If you need a response in less than two (2) business days please contact your clinician's office directly by telephone. Our regular business hours are Monday through Friday 8 a.m. to 5 p.m. For all urgent medical matters, contact your clinician's office.
- You agree that MyHealth is a communication service offered as a convenience to Harvard Vanguard Medical Associates patients. You understand that it provides you with online access to portions of your Harvard Vanguard Medical Associates electronic medical record and that you are solely responsible for any sharing of MyHealth: Your Personal Health Connection content that you intentionally or unintentionally communicate to others.
- You understand that messages will be reviewed by your healthcare team, including Medical Assistants, to determine who can best respond to your message. Your message will be routed to the appropriate individual and/or department for handling, and therefore, Harvard Vanguard Medical Associates employees, other than your healthcare provider may be involved in addressing your request. If you only want a particular clinician to receive the information you should not send such information through MyHealth . Remember, use of MyHealth may not be appropriate if there is sensitive information that you would only wish to discuss directly with your provider.
- You understand that you may receive messages, test results and other communications delivered via MyHealth. If you receive this information through MyHealth, in general you will not receive separate written communication regarding this information from your health care team at Harvard Vanguard Medical Associates.
- You understand that messages you send via MyHealth may be made part of your permanent medical record at the discretion of the clinician. Once it is made part of your record, it will be accessible to current and future Harvard Vanguard Medical Associates staff members who are appropriately involved with your care.
- You understand that not all of your Harvard Vanguard Medical Associates medical record will appear on your MyHealth. Posting of information is strictly at the discretion of your individual clinician.
Security
All communications between you and your Harvard Vanguard Medical Associates health care team and access to your personal health information using MyHealth are carried over a secure, encrypted connection directly into a segregated section of the Harvard Vanguard Medical Associates electronic medical record. While we use state-of-the-art security, no system can perfectly guard against risks of intentional intrusion or inadvertent disclosure of information. When using Harvard Vanguard Medical Associates My ChartTM, information will be transmitted over a medium that is beyond the control of Harvard Vanguard Medical Associates and its contractors. YOU HEREBY EXPRESSLY ASSUME THE SOLE RISK OF ANY UNAUTHORIZED DISCLOSURE OR INTENTIONAL INTRUSION, OR OF ANY DELAY, FAILURE, INTERRUPTION OR CORRUPTION OF DATA OR OTHER INFORMATION TRANSMITTED IN CONNECTION WITH THE USE OF THIS SERVICE. Once the information is received by us, your medical information will be treated as confidential and given the same protection that all other Harvard Vanguard Medical Associates medical records are given. While you may receive Internet e-mail messages notifying you of new messages in your MyHealth Inbox, these e-mails will not contain any confidential medical information.
When you initially enroll to use MyHealth, you will be provided a temporary access code. It is extremely important that you keep your MyHealth temporary access code, user ID and password in a secure location and completely confidential. Anyone with access to your temporary access code, user ID and password will be able to view your medical information and communicate with your health care team as if that person were you. It is your responsibility to prevent disclosure of your temporary access code and password and to change your password if you feel that your security has been compromised. You can change your password in MyHealth at any time by choosing "Change password" under the administrative options.
Confidentiality
Your login name and password are key to protecting the confidentiality of your information. You are responsible for keeping your password confidential. It is only known by you and can only be changed by you online. Your password is not known by anyone at Harvard Vanguard Medical Associates.
As we provide your health care, we are required to maintain documentation of your medical history, current conditions, treatment plan and all treatments given, including the results of all tests, procedures and therapies. Messages you send via MyHealth may be made part of your permanent medical record at the discretion of the clinician. Once it is made part of your record, it will be accessible to current and future Harvard Vanguard Medical Associates staff members who are appropriately involved with your care.
Harvard Vanguard Medical Associates is deeply committed to protecting our patients rights to privacy, and to safeguarding patient information. We have detailed policies and procedures regarding access to all records by our workforce. These policies conform to state and federal law and are designed to safeguard your privacy.
Our workforce is educated in Harvard Vanguard Medical Associates confidentiality policies and the appropriate use of medical information. A Harvard Vanguard Medical Associates patient's medical information is held in strictest confidence.
All Harvard Vanguard Medical Associates generated electronic medical record content is subject to all state and federal statutes governing the security and confidentiality of medical records.
Discontinuing Use of MyHealth
You may discontinue your use of MyHealth™ at anytime by sending a secure message to your PCP's office requesting this change.
You agree that you will not post any offensive material on this site. If we determine that you have violated this agreement and/or abused the use of this service, we reserve the right, at our sole discretion, to discontinue your use of this service. You will be notified if your MyHealth™ service is discontinued.
Your MyHealth service will also be discontinued if you no longer have a primary care provider who participates in MyHealth at Harvard Vanguard Medical Associates.

All content included on Harvard Vanguard Medical Associates websites, including, but not limited to, text, photographs, graphics, button icons, images, artwork, names, logos, trademarks, service marks and data (the "Content"), in any form including the compilation thereof, are protected by U.S. and international copyright law and conventions.
Site Access and Licenses
Harvard Vanguard Medical Associates grants a limited license to each user to make personal use only of the website and the associated services in accordance with these terms and conditions of use.
